Continuity of care can be improved through smooth information flow and digital solutions

According to a recent report, continuity of care can be improved by developing information management, harmonising documentation practices and making wider use of digital solutions and artificial intelligence.

The aim is for the population to be assigned to named doctors as automatically as possible, for client contacts to be seamlessly directed to the client's own professional, and for care-related information to be available in real time across different services.

Consistent information management and documentation practices are also essential for knowledge-based management and national monitoring.

The report identified areas for development

According to the report, current information management practices are partly fragmented, and information does not flow sufficiently smoothly between different actors. There is variation in documentation practices, which hampers the use of information both in client work and in monitoring of operations.

The following areas for development were identified:

  • development of tools and data structures for population distribution,
  • structured recording of named doctor and named nurse information in the Kanta services,
  • automatic routing of client contacts to the client's own professional,
  • strengthening the use of health and care plans, and
  • use of digital solutions and artificial intelligence in support of continuity of care.

Wellbeing services counties can draw on the report's proposals in system development, procurement and the renewal of operating models.
