Joint Action on Cardiovascular Disease and Diabetes (JACARDI)
In the JACARDI project, the aim is to develop the prevention and treatment of cardiovascular diseases and diabetes, as well as promote participation in working life despite sickness, by implementing previously proven effective or promising models based on evidence according to national needs. The project involves 21 European countries.
Objectives
The main objective is to manage the effects of cardiovascular diseases and diabetes at the societal, healthcare system, and individual levels, paying particular attention to sustainability, equity and accessibility aspects. The goal is for the operational models to remain in use after the project period.
Implementation
The thematic areas of the JACARDI project include registers and databases, health literacy, risk identification and prevention, care pathways, self-management, and participation in working life. In total, there are 142 operational models ("pilots") to be tested in the project, and their national implementation will be carried out following a systematic development model in collaboration with national stakeholders. The project considers the ethnic and cultural diversity of the population as a cross-cutting theme.
THL (Finnish Institute for Health and Welfare) leads several thematic areas at the project level. In addition, THL, together with partner organizations, will implement one or more pilots in each thematic area.
