Functional capacity of older people, living at home and support for informal carers

Health promotion funding is granted to regional projects that develop practices for promoting the health of older people and informal carers. Finnish Institute for Health and Welfare (THL) supports the funded projects and studies their impacts.

Goals

THL's objective is to support the development and embedding of operating models and structures that promote the wellbeing of older people and informal carers in wellbeing services counties and municipalities.

The aim is to produce new knowledge on the current state of health and wellbeing promotion among older people and informal carers, effective practices and the impacts of activities, in order to promote their health and functional capacity and, based on evidence, slow the growth in the need for more intensive social and health care services.

Implementation

The task of THL is to

  • prepare the priority areas related to older people and informal carers for the health promotion funding call, support applicants in preparing their applications and assess funding applications.
  • coordinate and support the funded projects
  • guide the development of support for informal care in wellbeing services counties through interactive guidance
  • examine the state of health promotion among older people and informal carers and issue recommendations on development needs
  • studie the impacts of projects launched in 2025 on clients' health, wellbeing, functional capacity and participation as well as regional cooperation structures for the promotion of wellbeing.

Key focus areas of the regional projects:

  • Implementation of the National Memory Service Path operating model
  • Supporting the wellbeing and health of older people by expanding and renewing group-based activities that promote functional capacity
  • Implementation of cultural content developed under the Cultural Wellbeing for Older People programme
  • Supporting equal opportunities for older people to participate, engage in active citizenship and pursue lifelong learning
  • Promoting the health of informal carers and strengthening their resources and capacity to cope
  • A support model for various demanding situations in informal care.
