Project page (JACARDI)JACARDI is a European Union co-funded, pan-European project that helps EU Member States reduce the burden of cardiovascular diseases and diabetes at both individual and healthcare system levels.
The project involves 81 partners from 21 European countries.
Goals
The main objective of the project is to manage the impact of cardiovascular diseases and diabetes on society, the service system, and individuals, with particular attention to equality, diversity, and the continuation of project outcomes after the project period.
Implementation
The JACARDI project focuses on six themes covering the entire patient pathway:
- Strengthening health literacy
- Improving access, quality, and availability of (registry) data
- Identifying individuals at risk of disease
- Developing integrated/seamless care pathways
- Supporting patient self-care
- Enabling participation in working life for people living with cardiovascular diseases or diabetes
The project will implement a total of 143 pilot projects in 18 European countries. The pilots will adopt previously proven or evidence-based promising models according to national needs. Planning, implementation, and evaluation will be carried out using a harmonized method in collaboration with local actors. Special attention will be paid to promoting equality and diversity and ensuring the sustainability of tested models beyond the project period.
Finnish Institute for Health and Welfare (THL) will lead several thematic areas and implement a total of 10 pilot projects in collaboration with the Finnish Diabetes Association, the Finnish Heart Association, and the University of Eastern Finland.
